1. In-Home Onboarding with Care Companion
- Your personal Care Companion comes to your loved one at home.
- They conduct an onboarding session, which includes:
- A review of medical history for the doctor
- An overview of the plan and what resources are available to your them
- How use the telemedicine app
- Talk about next steps and what to expect from the doctors visit
2. Initial Home Assessment by a Doctor
- A certified doctor visits your loved one at home.
- They review medical history, current medications, daily routines, and home safety.
- Together, they discuss goals—like “fewer clinic visits,” “better blood pressure control,” or “more stability when walking.”
- The doctor documents a personalized care plan, including immediate goals that can be addressed with the help of the Care Companion
3. Nurse Visits for Ongoing Monitoring
- A nurse comes to the home for regular check‑ins. Each year, a family can expect two in-person visits and two virtual phone calls, conducted every 3 months.
- They measure vital signs, check symptoms, review medications, and ask about any new concerns.
- If something needs attention, the nurse can escalate to a doctor or help schedule follow‑up care.
4. Care Companion Support
- When a clinic or hospital visit is needed, a Care Companion:
- Helps schedule the appointment
- Provides by‑their‑side support at the visit
- Helps explain next steps and coordinate care with trusted partners
5. 24/7 Access To Doctors Between Visits
If something comes up between in‑person visits, your family has 24/7 access to certified Philippines‑based doctors via virtual consultation.They can triage concerns, give advice, and decide whether an in‑home or in‑clinic visit is needed.
6. After-Visit Summaries
After each visit, you receive a clear summary of what occurred in the visit.You see what was done, what was found, and what’s next—so you can stay in the know and fuly support from the US.
